The easy answer is, qmail is a really fast and reliable email server / mail relay. The most common setup for me is to place qmail in the DMZ as a reverse proxy for incoming email, I add recipient filtering, spamfilters and anti-virus. I also usually set up reverse pop / imap / smtp / http proxies to expose services out side the firewall for your corporate email server.
